 1. Provide two current proofs of residency.

Examples of acceptable documentation:

Lease/rent agreement

Real estate contract/proof of home ownership

Cable television bill

phone bill

Electric/ Gas bill

2. Additional Materials Needed for registration

:

Copy of an official/government issued birth certificate

                           Copy of immunization records

                           Copy of physical examination form

                           Report Card (previous school

                          WITHDRAWAL form(previous school
